Providence is calling a Certified Nursing Assistant Hospice Aide - Hospice East Portland (1 FTE, Days Full-Time) to Providence Home Services Oregon in Portland, OR.

We are seeking a Certified Nursing Assistant Hospice Aide - Hospice East Portland that will be under the supervision of a registered nurse, provides supportive personal care services to patients of all ages according to the individualized physical needs of the patient in accordance with state and federal regulatory requirements.

In this position you will have the following responsibilities:

• Provide personal care to hospice patients consistent with their Plans of Care under the supervision of a Registered Nurse

• Provide homemaker duties as defined by Medicare e.g. cleaning, organizing and food and water preparation consistent with the Plan of Care

• Report changes in the patient's condition or living situation as they relate to personal care needs

• Complete documentation in the patient medical record following established policies and procedures

• Meet mandatory Federal requirement of twelve hours of in-service education per calendar year

• Participate in Interdisciplinary Team conferences

• Contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities

Required qualifications for this position include:

• Education to meet certification, license -O R- registration requirement

• Current State of Oregon CNA License

• Valid driver's license

• Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) within thirty (30) days of hire

• Two (2) years of experience (exception would be a nursing student with current CNA)

• Experience using electronic medical record software
