Providence is calling a Registered Nurse (RN) Infusion Coordinator (1.0 FTE, Day Shift) to work in the Patient Registrationat Providence Infusion and Pharmacy Services in Tukwila, WA

The Registered Nurse (RN) Infusion Coordinator will serve as member of the infusion team, responding to incoming referrals and informational calls regarding outpatient / home infusion services. The Intake Coordinator is responsible for collecting and reviewing clinical information in order to ascertain the appropriate services for the individual referred. This position is responsible for assisting the patient, referral source and any other callers with infusion services. This person will facilitate the coordination of services with the patient, from the initial referral through the duration of therapy and discharge from infusion services. This position acts as a communication link between Providence Infusion Pharmacy, clinicians, patients, families, and physicians by creating a user-friendly point of entry to discuss questions, problems or concerns throughout the continuum of care.

As the RN Infusion Coordinator it is the expectation of this position to work closely with the Intake Infusion Pharmacist to guarantee that services and care provided to patients is appropriate based on the patient's clinical needs. It is also the responsibility of the RN Infusion Coordinator to educate and delegate responsibility to the other team members within their group. The RN Intake Coordinator will be responsible for working with their group to ensure optimal customer service to, patients, families, clinicians, discharge planners and physicians.

In this position you will:

  • Respond to incoming calls from patients, families, physicians, and referral sources:

  • Provide general information about home infusion services in response to inquiries

  • Collect sufficient and appropriate referral information to determine a prospective patients service needs. For private insurance patients, provides clinical interpretation of patients needs, with insurance case manager

  • Assign and coordinates referrals to the appropriate nurse, taking into consideration caseload availability, geographic areas, staff skill level and expertise

  • Assign and coordinates referrals to outside nursing agencies

  • Is responsible for the completion of the Delegation/Coordination of Care and Plan of Treatment (when applicable)

  • Maintain close communication with Nurse Scheduler regarding patient visit needs

  • Triage calls from patients and other customers: Participates as resource for Nurse Clinicians

  • Maintain close communications with Intake Pharmacist and Dietitians regarding clinical needs of patient

  • Maintain close communications with Hospital Liaisons, hospital based Nurse Clinicians, Hospice Coordinator and other referral sources in relationship to issues around patient referrals, discharge plans, and visit needs

  • Maintain close communications with Admission Assistants for authorizations and updates

  • Contribute to Providence Infusion Pharmacy function, though effective time management, utilization of problem solving skills to address issues:

  • Appropriately utilizes Manager of Nursing or other Administrator for guidance/problem solving

  • Maintain professional appearance

  • Attend mandatory in-services

  • Participate in identifying topics for in-services

  • Complete all competencies required by Nurse Clinicians and will be competent to see patients if needed

  • Participate in new staff orientation and training as appropriate

  • Participate in agency quality improvement activities and improvement of care activities by gathering data for QA/I, identifying issues and implementing recommended changes as appropriate

  • Safeguard and maintains confidentiality of all patients, medical staff, employees and competitive information related to home infusion or any business of Sisters of Providence

  • Maintain current knowledge of computer system and is able to input and retrieve patient data as needed

  • Support and incorporates the Mission into daily activities

  • Communicate clearly, sensitively and in a supportive professional manner with patients/family/caregivers, co-workers and the public:

  • Seek ideas, opinions and feelings of patients/family/caregivers and co-workers

  • Express self in a calm, clear, confident manner in verbal, non-verbal, and written communication

  • Demonstrate awareness of own views/ values/beliefs and works to minimize any limiting/negative impact on patient care and professional relationships

  • Assure appropriate patient care documentation, use of standard forms and standardized patient teaching materials

  • Assist with development and maintenance of systems for communication with pharmacy, billing, Home Health, Hospice, hospital staff and physicians

  • Participate in problem solving patient care issues with nursing staff

Job Requirements

Qualifications:

Required qualifications for this position includes:

  • Graduate of accredited School of Nursing

  • Current Washington State Registered Nurse license

  • Cardiopulmonary resuscitation (CPR) certification

  • Two (2) years of Direct clinical experience, preferably with one year of home infusion field experience

  • Thorough knowledge of and proficiency in nursing principles, process, and procedures

  • Effective problem solving, communication, and organizational skills

  • General home care knowledge and skill

  • Customer service skills

Preferred qualifications for this position includes:

  • Bachelor's Degree in Nursing

  • One (1) year of office experience in health care setting

  • Computer experience with infusion software, electronic medical records (EPIC), Microsoft Word, and Excel

  • Knowledge of the practice of IV therapy
