Providence is calling a Licensed Practical (LPN) or Vocational Nurse (LVN) Hospice (Per Diem/Evening Shift) to Providence TrinityCare Hospice in Torrance, CA
We are seeking a Licensed Practical (LPN) or Vocational Nurse (LVN) Hospice who will be responsible for providing direct patient/family care under the supervision of a Registered Nurse for the Hospice Care Program. Responsibilities include following the Plan of Care, providing treatments, and working collaboratively with the members of the team to help meet positive patient/family care outcomes. May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
In this position you will:
Provide direct patient care as defined in the State of California Nurse Practice Act
Implement current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse
Provide patient/family/caregiver education and information pertinent to diagnosis and Plan of Care
Participate in coordination of home-based palliative care services, appropriately reporting the identified needs for the Interdisciplinary Group (IDG) members, e.g., Certified Home Health Aide (CHHA), OT, PT, MSW, ST, Dietitian, Chaplain to the clinician or Patient Care Manager
Provide appropriate pain/symptom management
Evaluate patient’s response to treatments/medications
Understand the hospice benefit and levels of care
Maintain productivity standards as set by organization guidelines
Use equipment and supplies effectively and efficiently
Documentation / Information Management:
Provide accurate and timely documentation of patient services to reflect the Plan of Care
Submit documentation for the patient medical record within the timeframe set by the organization guidelines
Leadership / Supervision:
Participate in orientation, education, and development of team members as directed by supervisor
Development:
Participate in appropriate continuing education as may be requested and/or required by immediate supervisor
Accept personal responsibility for other educational activities to enhance job-related skills and abilities
Performance Improvement:
Participate in the Performance Improvement Program and participate in and adheres to the PH&S/TrinityCare Hospice Corporate Integrity Plan
Budgetary:

Qualifications:
Required qualifications for this position includes:
Graduate of a Vocational Nursing program
Currently licensed as an LVN in the state of California
Possesses and maintains current CPR certification
Must be a licensed driver with an automobile that is in good working order and insured in accordance with state and/or PH&S/TrinityCare Hospice requirements
Obtain IV certification within the six (6) months of hire
One (1) year Nursing experience
Comply with accepted professional standards and practice
Demonstrate good verbal and written communication and organizational skills
Demonstrate understanding of hospice philosophy and concept and understands the needs of the terminally ill
