We are seeking a Nurse Assistant - Nursing pool role which is a non-licensed health care employee who performs delegated routine and predictable patient care tasks/duties and works under the direct supervision of a Registered Nurse (RN). These duties may include, but are not limited to EKGs, vital signs, basic patient care and transportation. Responds appropriately to patients and families with knowledge of age-specific needs based on scope of service of unit/department where assigned..
In this position you will:
Checks patient’s I.D. band daily for accuracy.
Assists with patient admitting/transfer:
Sets-up room, including monitor equipment, if ordered. Orients patient to environment and routine.
Problem solves and serves patient needs.
ares for/labels/stores patient personal property,
Labels room and equipment to identify by patient.
Observes and reports data collection to RN.
Documents
Assists during discharge:
• Participates in discharge activities, as directed
• Accompanies patient to transportation or transports.
Provides direct patient care tasks/duties that have been delegated by the RN. Recognizes and reports problems immediately to the RN:
T akes vital signs, weight, etc., as assigned.
Changes simple dressings (non-sterile).
Performs/records finger stick blood sugars.
Performs pulse ox screening and records.
Applies, changes, removes external catheters.
Performs pre-surgical preps, as needed (i.e. shave).
Ambulates, repositions patient and performs range of motion (ROM).
Applies, changes patient restraints according to established policy.
Applies K-pads, cold packs and hyper/hypothermic blankets.
Empties Jackson-Pratts and Hemovacs.
Applies anti-thromboembolytic hose, sequential compression devices and Plexipulse.
Calculates fluid intake and output, calculates % meal eaten.
Empties and changes drainage containers, reports and records contents.
Performs specimen collection – NG, emesis, stool, and urine.
Performs tray/supply set-up for a bedside procedure.
Performs monitoring activities, reports, documents, (i.e., dynamap, pulse ox).
Provides post mortem care.
Assists with tube feedings under the direct supervision of the RN, NOT INITIAL FEEDING.
Assists in complex feeds.
Assists in IV set-up (gather supplies, pump, etc.).
Incentive Spirometry follow-up and coaching.
Assists with orthopedic equipment/applications, as directed.
Performs tap water enemas and Harris flushes.
Personal hygiene:
Bathes patient or assists.
Provides back care, including massage.
Provides oral hygiene and dental care.
Provides hair care, including shampoo and brushing.
Provides nail care on hands and feet.
Provides Foley catheter care.
Shaves patients.
Performs transportation of patients, lifting support, as assigned.
Recognizes emergency situations; promptly reacts; initiates or supports CPR; follows directions appropriately in emergency situations; reports observations.
Leadership and Supervision:
Acts as a team problem solver by being an active member of unit/staff meetings and other committees.
Assist with the orientation and on-the-job training of new hires and acts as a buddy to (SCROC) students.
Understands and is responsible to perform within general scope of practice.
Documentation/Information Management:
Responsible for timely, accurate and concise documentation and order entry.Enters physician orders, as directed.
Obtain results on diagnostic data, files appropriately.
Enters requisitions.
erforms documentation and records: Vital signs, weight, container contents (drainage), hemocults, fluid intake and output, and % of food eaten, and all patient interventions.
Utilizes hand held computer for documentation.
Demonstrates working knowledge of Hospital Information Systems.
Communicates patient information and changes to RN.
Maintains confidentiality of all patient and hospital related information.
Performance Improvement:
Participates in and can articulate department and hospital performance improvement projects.
Budget Responsibility:
Responsible to use equipment and supplies in a prudent and efficient manner.
Identifies opportunities for cost containment.
Supports emergency staffing needs.
Qualifications:
Required qualifications for this position include:
Graduate of any accredited Certified Nursing Assistant program, or Nursing Assistant program or equivalent educ/experience
Completion of 2nd semester of accredited Nursing Program, HHA with 1-2 years experience
A current BLS Card from the American Heart Association for Health Care Providers or American Red Cross CPR / AED for the Professional Rescuer
Ability to perform basic arithmetic computations (add, subtract, multiply, divide)
Ability to perform all physical requirements pertaining to delegated tasks
Ability to accurately and efficiently complete assigned tasks
Ability to organize and prioritize work to complete assignments in a timely manner
Must demonstrate an understanding of basic medical terminology and abbreviations
Ability to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families and co-workers
