The Thoracic Nurse Navigator will work collaboratively with Knight Cancer teams to include Physician, surgeons, residents, fellows, and RN coordinators in the care of this complex population throughout the continuum of care and treatment. This position will work with the clinical nurse manager and other oncology nurse navigators to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in determining and outlining the plan of care. This position will collaborate with the other navigators to cover day off and vacations. An Oncology Nurse Navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The ONN will work with the ONS core competencies as the guide. The ONN competencies to be performed: (a) coordination of care, (b) communication to patient, families, and members of the healthcare team, (c) patient and family education (d) professional role .

Orientation: 6-8 weeks
