Qualifications

Education

Completion of State Approved LPN Training Program is required

Experience

1 year of acute nursing experience is preferred

Prior experience in home health or hospice is preferred

Certifications, Licenses and Registrations

Basic Life Support (BLS) certification is required

License as a Licensed Practical Nurse (LPN) in the state of practice is required

Valid state driver’s license, current automobile insurance and reliable automobile is required.
