The Patient Care Coordinator (PCC) is a registered professional nurse who leads and coordinates the delivery of patient care in the Emergency Department. The PCC collaborates with the Nurse Manager and other team members to develop and implement patient care policies and procedures, monitor, and evaluate patient care outcomes, and provide mentorship and support to staff nurses. The PCC also demonstrates leadership skills in the coordination and delivery of patient care, communicates effectively with members of the health-care team, patients, and families, and coordinates and collaborates with other team members to facilitate effective throughput on the unit.
Essential Duties and Responsibilities
Leads and coordinates the delivery of patient care in the Emergency Department.
Develops and implements patient care processes in collaboration with the Nurse Manager and other team members.
Monitors and evaluates patient care outcomes.
Provides mentorship and support to staff nurses.
Demonstrates leadership skills in the coordination and delivery of patient care.
Communicates effectively with members of the health-care team, patients, and families.
Coordinates and collaborates with other team members to facilitate effective throughput on the unit.
Organizes and directs the delivery of patient care services on the assigned unit in collaboration with the Nurse Manager and Nursing Director, ensuring that appropriate levels of care are provided to patients based on acuity.
Coordinates unit staff breaks, and coverage as needed.
Supports and participates in the orientation of new personnel to the unit.
Performs narcotic counts per St. John's Hospital policy.
Checks crash carts and dietary refrigerator temperature log daily.
Participates in environmental rounds with the Nurse Manager and ANM.
Communicates pertinent patient and administrative information to team members on all shifts.
Monitors and assures compliance with all regulatory protocols, including but not limited to stroke, sepsis, cardiovascular (HEART score), and notifications.
Non-Essential Duties and Responsibilities
May perform unrelated duties assigned.
May assist in emergency situations.
Physical Requirements of the Job
Sits, stands, bends, lifts, and moves intermittently during working hours.
Must be able to lift, push, pull, and move a minimum of thirty-five (35) pounds.
Must be able to see and hear or use prosthetics that enable adequate sensory function to assure that the job responsibilities can be met.
Must have good hand-eye coordination.
Current NYS license and registration as a Registered Nurse. BSN degree preferred. Minimum of three (3) years experience as a staff nurse in an emergency department. BLS, ACLS, PALS required. CEN/CPEN Certification highly preferred and must receive within 18 months. Charge Nurse experience preferred. Active participation in ongoing professional development. Effective communication and time management skills required
