STGi is currently seeking a qualified Licensed Practical Nurse Case Manager candidates for the Department of Housing and Urban Development (HUD) and Veterans Affairs (VA) Supportive Housing (HUD/VASH) Program.
The Licensed Practical Nurse Case Manager will provide case management and will support the coordination of the appropriate medical and mental health services for Veterans in the program. The LPN will provide direct nursing care as needed.
ESSENTIAL FUNCTIONS:
Clinical Services and Supports:
Work directly with HUD/VASH Clinical Supervisor and HUD/VASH Treatment Team to provide comprehensive discipline-specific services to Veterans and their families.
Work with case managers to develop appropriate treatment plans which will focus on the Veteran’s health and wellness as well as maintenance of permanent, stable housing.
Provide case management to participants, as needed, throughout the continuum of care.
Provide individualized and/or family education and/or support groups with a focus on self-management, prevention, and wellness to ensure the Veteran’s continued success.
Participate in discharge planning with other treatment team members and maintain responsibility for discharge plans including timely and appropriate execution.
Refer participants and family members to resources for coping and dealing with the loss and grief experiences in disability, terminal illness, and death.
Provide outreach services to engage homeless Veterans, especially those who are chronically homeless with complex medical and mental health needs.
Assist in the navigation of healthcare systems including providing referrals or advocacy to support the achievement of patient outcomes. This may include accompanying Veterans to appointments to facilitate information sharing.
Assist Veterans by simplifying their treatment recommendations and medication regimens to promote Veteran agreement and cooperation to enhance positive treatment outcomes.
Quality Assurance and Reporting:
Collect and submit participant data per contract requirements.
Document interventions and activities in participant’s clinical record.
Required Skills
Ability to work collaboratively with others and in a team.

Uses good time management skills and resources to balance case load direct service and paperwork.
Excellent verbal and nonverbal communication skills.
Must have a current California driver’s license and insurance for regular driving and transporting.
Required Experience
Graduate from an approved, accredited Licensed Practical Nurse program located in the state of CA.
Current, full, and unrestricted LPN license in the state of CA.
One year of professional experience as a licensed practical nurse.
BLS for Healthcare Providers® certification through the American Heart Association®.
Experience working with the homeless population preferred.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
The position requires visual and auditory acuity at a level to provide safe and effective nursing care.
Ability to distinguish shades of colors.
Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds.
Possible exposure to inclement weather during visits with veterans in their community
Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty.
Additionally, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise, and STGi has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
