Job Description

A healthcare system in New York City is currently seeking a licensed Registered Nurse RN) to join their growing Primary Care team in the Bronx.

About the Opportunity:

  • State Date: ASAP
  • Assignment Length: Ongoing
  • Specialty: Primary Care
  • Schedule: Monday to Friday (rotating late Wednesdays & Saturday day shifts)
  • Hours: 8am to 5pm (12pm to 8pm on Weds; 8:30 to 3pm on Sat)

Job Requirements

Responsibilities:

The Primary Care Nurse (RN) will:

  • Perform both clinical and administrative functions by providing high quality patient care and direction for optimal unit operations
  • Handle patient screenings; triage; pain management; falls prevention, nutritional and psychosocial, discharge planning and other interventions to restore, maintain or promote patients’ physical health
  • Assist all patients, families/significant others through the healthcare system by acting as a patient advocate and navigator
  • Prescribe, delegate, and coordinate all care provided to assigned patients
  • Execute medical plan of care, at the direction of the treating clinician
  • Support patients’ self-management of disease and behavior modification interventions
  • Coordinate continuity of patient care with external healthcare organizations and facilities, including the primary care provider to the specialty care provider
  • Participate on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to Patient-Centered Medical Home and performance improvement initiatives
  • Assist in the prior authorization process for clinical and pharmacy related orders
  • Perform other duties, as needed

Qualifications:

  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • BLS certification
  • Experience working with he Adult and/or Pediatric population
  • Phlebotomy experience
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • 2+ years of experience in an Outpatient roles/setting
  • Bachelor's Degree in Nursing
