The Licensed VocationalNurse (LVN) assumes responsibility and accountability for theapplication of the nursing process and the delivery of patient/client careand/or contracted services under the supervision of aregistered nurse or licensed designee. The LVN provides service in accordancewith Maxim philosophy, standards and policies.
Essential Duties andResponsibilities:
1. Utilizes the nursingprocess to plan, implement, and evaluate patient/client care and/or contracted services
2. Demonstrates on eachassignment the provision of skilled nursing care rendered in accordance with prescribed orders
3. Identifies anddocuments changes in patient’s both physiologic and psychosocial condition
4. NotifiesDirector/Supervisor if applicable on the following;
a. Physiciannotification to obtain necessary orders for intervention(s) per state regulations
b. Changes inpatient/client condition
5. Adheres to Maxim'spolicies and procedures and applicable protocols
6. Identifies andprioritizes actions based upon patient/client care requirements
7. Assures patientsafety at all times and recipient of services provided
8. Verifies thatdocumentation reflects patient/client collaboration, participation in plan ofcare, expected patient/client outcomes, and selectednursing interventions
9. Performsinterventions, treatments, and medication administration, according toidentified priorities, plan of care, facility policies andpatient/client care outcome standards as applicable.
Monitors and documentseffectiveness of interventions, treatments, and medications
10. Evaluatespatient/client and provides a safe, comfortable and therapeutic patient/client environment
11. Monitors the safetyand effectiveness of the environment and equipment including universal precautions
12. Participates inactivities to facilitate good interpersonal communications
13. Communicatesobservations and significant changes in patient/client status to the Directorof
Clinical Services and/orClinical Supervisor /Case Manager or licensed designee
14. Documents allphysician collaboration/coordination of care in the medical record and communicates this information as needed
15. Attends caseconferences, team meetings, education in-services as required
16. Supportsprofessional practice by observing the following;
a. Nurse Practice Act
b. Standards of NursingPractice
c. Standards of PatientCare
d. Standards ofAccrediting Body (ACHC, Joint Commission, etc.)
e. State and Local Laws
f. Patient Bill ofRights
g. Maxim Healthcare,Nursing, and Client’s Hospital Policies
17. Performs otherduties as assigned
Minimum Requirements:
1. Graduate of anaccredited vocational nursing program
2. Licensed VocationalNurse in specific state
3. Complies withaccepted professional standards and practice
4. At least one yearprior professional nursing experience, preferred
5. Willingness toparticipate in skills advancement training or novice nurse program ifapplicable
6. Current CPR as applicable,for direct patient care
7. PPD or chest x-ray ifapplicable
8. Successful completionof new hire training as applicable to job site
9. Participation andcompletion of Maxim’s Competency Program
10. Strong verbal andwritten communication skills
11. Current HealthCertificate (per facility contract or state regulation)
12. Must meet allfederal, state, and local requirements
