Join the team of a rapidly growing, start up healthcare organization as a Nurse Practitioner!

This is a full-time position for an NP who resides in any of the following counties: Martin County, Bertie County, Washington County, or Edgecombe County.

We're seeking a Nurse Practitioner to provide exceptional, personalized care to cohorts of medically complex, underserved Medicare/Medicaid patients. In this role, you'll provide patient care in a variety of settings- clinic, mobile airstream clinic, home, and video. You'll partner with Galileo’s clinical specialists around conditions that have the greatest impact on the patient population—particularly behavioral health. 

This position acts as a key leader on the clinical team, which provides exceptional care to cohorts of complex Medicare/Medicaid patients. She or He drives ongoing improvements for the organization's care model, optimizing outcomes while decreasing inefficiency, and will utilize and inform the development of new digital tools for patient communication, team collaboration, and clinical decision support.

Salary: Market Rates / DOE, plus full benefit package

Job Requirements

Required Skills and Experience:

At this time, employees in this role are required to have completed a first and final dose of a COVID-19 vaccination approved or authorized for emergency use by the FDA, consistent with applicable law.

  1. MSN degree or higher
  2. Valid NP certification/license in NC
  3. Current residency in Martin County, Bertie County, Washington County, or Edgecombe County.
  4. 2 - 4 years experience with any of the following: Primary Care Setting and/or Geriatrics
  5. Active Nurse Practitioner with 2-4 years' broad clinical experience caring for patients with complex chronic illness
  6. Skill at navigating behavior change in the face of mental illness and social needs
  7. Proven experience with successful behavior modification
  8. Comfort with basic procedural skills, particularly vaccination and phlebotomy
  9. A collaborative, empathetic approach to leading care teams that fosters effective and efficient interventions, as well as inspires care team members to achieve ambitious goals and further their own development
  10. Strong “builder” mindset, with the ability to continuously assess how our care model can improve as it scales
  11. An ability to understand and prioritize patients’ core needs and translate them into well-designed, actionable care plans
  12. Already authorized to work in the US for any employer
