Responsibilities
Below is a list of your responsibilities as an Regional Re-Entry Coordinator with YesCare:

Ensures a nursing standard system for continuum of care for reentry that begins at admission to custody and linking soon to be release patients with appropriate community resources for seamless transition

Provide direct clinical supervision to the on-site RN discharge planners statewide
-
Collaborate with the Statewide Director of Nursing and work in partnership on nursing protocols as it related release-discharge planning.

Ensures data is collected from each discharge planner monthly for required reports by contract
-
Ensures release planning database is updated in real-time with all the required fields for accuracy
-
Works collaboratively with Director of Reentry to build collaborative relationships with organizations and providers in the community
-
Works collaboratively with the medical and mental health staff, DPSCS case management, DPSCS social work and the Facility reentry teams , to achieve the goal of a successful transition and re-entry to the community.

Educates discharge planners statewide to utilize a multi-system needs assessment that documents external linkages and matches the needs with available community supports.

Utilizes electronic databases and other resources of community healthcare and behavioral health providers for the purpose of facilitating discharge planning for the successful linkage

Monitors discharge-planning activities and results through routine audits related to nursing documentation and follow through

Reviews release medication report(s) and confers with Director on discrepancies and provides solutions to correct discrepencies

Maintain confidentiality and other standards as set forth by HIPAA, NCCHC, and other State and Federal guidelines.

Develops strategies to improve access to healthcare for returning citizens.

Collaborates with the CQI team to focus on reentry efforts and ongoing evaluation of reentry program.

Participates in any committees/meetings as deemed necessary by YesCare
-
Maintains open communication with all YesCare employees on all levels, multi-vendor and DPSCS reentry teams.

Co-Chairs reentry-discharge planner weekly huddles and meetings as required by contract

Conducts interviews and is required to be involved with on-boarding new reentry discharge planning staff
-
Works closely with discharge planning staff statewide for documentation education and troubleshooting
-
Follows standard precautions using personal protective equipment as required.

Experience & Requirements
-
Bachelor’s Degree of Science in Nursing, Master’s degree preferred
-
Experience in case management, discharge planning and providing services to a physically and mentally ill population preferred. Prior corrections or re-entry experience is also desirable.

Experience using Excel, Word, and Outlook.
-
Effective time management skills.
-
Subject to initial and ongoing security clearance requirement.
A background check, security clearance and drug screening are required for this role.
