Position Summary (This position will mainely cover the Lincoln County area)
The Hospice RN provides nursing care and coordinates care for terminally ill patients of all ages in their homes utilizing nursing principles that recognize the psychosocial, economic, cultural, spiritual and individualized health status and family/support systems and resources that affect health status and results in maximum outcome achievement. The Hospice Nurse serves in a supervisory capacity by directing and evaluating paraprofessional staff. The Hospice Nurse plans, provides, evaluates, coordinates care and provides case management and documents in a manner which meets the quality, efficiency, and care standards identified by MaineHealth Care at Home, as outlined in Policies and Procedures and according to professional standards of home health nursing practice. The Hospice RN acts as case manager and is responsible for overseeing the ongoing care planning management, coordination of care, communications and for outcomes of the plan of care. The Hospice RN acts to uphold the Agency’s mission, clinical expertise and maintenance of standards of care for Hospice and Palliative care and fiscal soundness within a changing health care environment.
Please note: This position requires weekend on-call, once every three weeks and one evening.
