733 Cedar St, Garberville, CA 95542, United States

Mission & Vision

Our Mission

To provide high quality local medical services, to engage community members with education, activities and lifestyle opportunities which promote optimal health, and to assist our patients in navigating access to services throughout the healthcare system.

Our Vision

To empower individuals to live longer, healthier lives through the use of information, relationships, and technology.

Core Values

  • Caring: We bring warmth and professionalism to all aspects of patient care.
  • Quality: We are committed to our patients, to our services, and to enriching the communities of Southern Humboldt.
  • Teamwork: We prioritize working together within our hospital as well as the larger community and healthcare network.
  • Positivity:  We believe in supporting an environment where healthy attitudes and respect fuel our culture.
  • Skilled Nursing Unit

Our eight-bed “distinct part” Skilled Nursing Unit (SNF) always has a waiting list. The nurse to patient ratio of 1 LVN to 8 patients is significantly lower than larger skilled nursing facilities where they can be 1:20-35 residents, enabling us to give individualized care to each resident. Our staff is a highly-skilled, committed team of licensed vocational nurses and certified nursing assistants, and is overseen by a registered nurse manager.

Our Skilled Nursing Unit is a highly-skilled, committed team of licensed vocational nurses and certified nursing assistants.

Each skilled nursing resident has an individual care plan to meet their unique needs. Depending on the resident’s diagnosis and care plan, we offer physical and speech therapy and we work with a registered dietitian to provide an individualized therapeutic diet.

Every Friday local musicians play for our residents. Friends, family, and community members often join in.
