Puget Sound Behavioral Medicine provides diagnostic and medical management services to children and adults with attention deficit disorder, mood disorders, and behavioral disorders of childhood. Services are provided by Theodore D. Mandelkorn, MD.

Since insurance coverage for mental health conditions is inadequate to cover the time-intensive treatment that we believe is essential for effective treatment of these conditions, we are not providers for any insurance programs and do not bill insurance companies. Payment is required at the time of service and can be by personal check or credit card. An itemized statement with the appropriate billing codes will be provided at each visit. Patients will need to submit these statements to their insurance companies for reimbursement. This policy applies to clinic visits and phone consultations.

Puget Sound Behavioral Medicine does not provide primary health care services. Patients need to have a primary care physician to take care of other health needs such as school and sport physicals, immunizations, and illnesses.
